Remove or suppress an address

All provider records here come from the public-domain NPPES registry. If your listed practice address is a private residence, you can ask us to suppress the street address on your profile. We verify each request before applying it.

What happens next

• Your request is logged and reviewed by a person — nothing is changed automatically.
• We aim to review requests within a few business days. We may email you at the address you provide if we need to confirm something.
• Suppression hides the street address, ZIP, and phone; the city and state remain, and the NPI record itself is public data we can't delete.
